What is FAPT?
The Family Assessment and Planning Team (FAPT) is comprised of the supervisory staff from the local Department of Social Services, School System, Community Services Board (mental health) and the Court Services Unit (juvenile justice), as well as a parent and representatives from private providers in the community. The FAPT works with the families to develop an Individualized Family Services Plan, or Plan of Care. This team makes recommendations for strategies and services based on the strengths and needs of the family.
